Intake Coordinator

SUMMARY

The Intake Coordinator is part of a multidisciplinary clinical team, conducting phone and face-to-face screenings and intakes on-site, off-site, and via telehealth for veterans, guardsman, active-duty, and their families. The Intake Coordinator will work with the team for case assignments, consultations, and care coordination.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Conducts initial intake assessments with clients.
Provides support to clients with completing initial consent forms and releases as required
Provides warm hand-offs for clients needing external referrals
Coordinates case assignments with multi-disciplinary team
Thoroughly and efficiently completes documentation within required timeline of 48 hours of intervention/communication
Collaborates with clinical team and participates in multidisciplinary team meetings
Provides crisis management support to clients and the team as needed
Provides services in-person and via telehealth platform, as required
Perform other duties as assigned
QUALIFICATIONS

Master’s degree in social work, Sociology, Psychology or related field is required; Oklahoma Licensure or Candidate for Licensure for one of the following is preferred:Licensed Professional Counselor (LPC)
Licensed Marriage and Family Therapist (LMFT)
Licensed Clinical Social Worker (LCSW)
Licensed Alcohol and Drug Counselor (LADC/MH)
At least one year of experience providing intake coordination and/or case management services; experience working with a military or veteran population is preferred
Knowledge of military connected and veterans service organizations and resources
Ability to think analytically and evaluate the impact of case assignments
Excellent written and oral communication skills
Highly organized, detail oriented, and reliable
Works effectively in and contributes to a healthy team environment
Proficient in use of technology to perform duties
Displays appropriate understanding of and adherence to policies and procedures and organizational hierarchy
Ability to set priorities and evaluate and create solutions to work related problems
Must have a valid driver’s licenses and reliable transportation
